£30.00 then two instalments of £75.00 by standing
order

Option 3 - Initial deposit of
£30.00 then four instalments of £40.00 by standing

Option 4 - Initial deposit of
£30.00 then six instalments of £30.00 by standing

finally by requesting a paper information pack to be sent out
to you again to your home by clicking the link below, please
remember to leave your full contact details including
postcode.

How To Complete The Standing Order
Mandate
Step 1
– Fill in section (1) with details of your banks full
name and address, please include the postcode
Step 2
– Fill in section (2) with details of your bank
account. Please give details of the full
name the account is held in, The sort code for the account and
the account number. When you have completed
all your bank details please sign and date the form at the
bottom of section (2) where shown.
Step 3
– Decide which payment option you would like from the
list above and then complete section (3).
Enter your first payment
to be dated before the start of the challenge (before Monday
15th October) in the ‘Date of first payment’
section. Next choose a date when you would
like the instalments to be taken out each month and enter this
in the ‘instalment date’ section. Post the
whole form back to us, we will then complete and send the form
to your bank and issue you with a photocopy.
Deposit
Please note we require a deposit of
£30.00 to secure your place on the challenge. This
deposit is a non returnable deposit and will only
returned if we cancel the challenge. This amount
has been deducted from the total amount payable on all
the options above.
Registration
Fee
All participants must
pay a registration fee of £25.00 on registration day which
covers all administration costs and initial and concluding
weight and progress monitoring sessions.
Please note if you do not wish to be entered in to the prize
fund and therefore do not want monitoring you do not have to
pay the £25.00 or attend the initial and concluding weigh
ins. This does not affect your right to
participate in the challenge.
Please note: Any cancellation of standing
orders before the total amount owed has been paid will
result in the debt being passed to a debt collection
agency
